Radiate Once More

The presented scenario of long-term oligo-(mono)metastatic disease from a biologically favorable subtype of breast cancer1 illustrates important lessons regarding effective local treatment. The safe and conservative reirradiation regimen (39.6 Gy) eventually failed to prevent symptomatic progression. Surgical intervention was required; however, this approach alone is unable to provide permanent local control. Therefore, we are facing the dilemma of thoracic spinal cord reirradiation (not included in the adjuvant, initial target volume) and nerve root re-reirradiation.
