Commentary: Toward less traumatic, not just less invasive, surgery

As surgeons, we are well aware of the magnitude of the traumatic stress response when we perform “big operations.” An esophagectomy is associated with a significant stress response; the fluid requirements following this operation is a telling consequence. In an attempt to decrease this traumatic stress response, surgeons have spent the last two decades trying to decrease the size of incisio ns and have moved toward minimally invasive laparoscopic or thoracoscopic and robotic resections and, more recently, toward enhanced recovery after surgery (ERAS) protocols based on available clinical evidence.
