GPs can tackle inequalities and reduce demand by 'focussing on small cohorts'

GPs and primary care networks (PCNs) could tackle health inequalities and reduce demand on practices by breaking down patient lists into smaller, manageable cohorts and looking at how their needs could be addressed, a leading GP has said.
