Intraventricular rupture of aspergillus brain abscess during remission induction

A 69-year-old man with newly diagnosed B-precursor acute lymphoblastic leukemia (B-ALL) had impaired consciousness (Glasgow coma scale [GCS] E4V4M6) on day 24 of remission induction therapy. Serum beta-D-glucan (61.0  pg/μL) and Aspergillus galactomannan antigens in the serum and cerebrospinal fluid (CSF) (both ≥5.0) were elevated. Computed tomography showed a nodule in the left lung. Magnetic resonance imaging (MRI) revealed multiple brain lesions, and brain biopsy confirmed aspergillosis (Fig. 1) accordin g to the updated invasive fungal disease definition (Donnelly et al., 2019).

We report that the bifunctional compounds enhance the interactions between primary human neutrophils and A. fumigatus in vitro, using three microfluidic assay platforms. The bifunctional compounds significantly enhance the recruitment of neutrophils, increase hyphae killing by neutrophils in a uniform concentration of drug, and decrease hyphal tip growth velocity in the presence of neutrophils compared to the antifungal targeting moiety alone. We present the case of a 3-year-old boy who was diagnosed with cerebral abscesses due to Aspergillus nidulans infection on day 28 of induction chemotherapy for acute lymphoblastic leukemia. He responded well to treatment with voriconazole and caspofungin, making a full recovery. There are very few cases of invasive aspergillosis reported in children during induction chemotherapy for acute leukemia and A. nidulans is rare in the absence of chronic granulomatous disease.

Aspergillosis is an opportunistic fungal infection that may develop in immunocompromised patients with conditions such as leukemia or aplastic anemia. A rare case of stomach perforation following acute fungal gastritis in a 13-year-old female patient with aplastic anemia is reported herein. We present a 19-year-old woman with active acute lymphoblastic leukemia who presented with several weeks of fevers and bilateral lower extremity weakness. She was found to have an intramedullary aspergillus abscess at T12–L1 resulting from adjacent vertebral osteomyelitis and underwent surgical debridement with ultra-sound guided aspiration and aggressive intravenous voriconazole therapy. Conclusions: An increasing trend in incidence of IA was observed during the latter half of study period. Early empiric therapy led to modest aspergillus-free survival.
