19-Year-Old Man With Fevers, Abdominal Pain, and Cough

A 19-year-old man with no significant past medical history presented to the emergency department for symptoms of fevers, cough, abdominal pain, nausea, and vomiting. He reported having cyclical fevers up to 103 °F for the past week. Four days before presentation, he developed nonlocalized abdominal pain and episodes of nonbloody emesis, not associated with any specific foods. He did not have diarrhea or constipation, and was passing flatus. He also developed a dry cough. Emesis was not associated with cou ghing fits.
