Optimization and auto-segmentation of a high risk cardiac zone for heart sparing in breast cancer radiotherapy

Breast radiotherapy is associated with an increased risk of ischemic heart disease, particularly for left-sided irradiation due to direct anatomical proximity between the left anterior descending coronary artery (LADCA) and the target volumes [1 –4]. First generation radiotherapy techniques, which relied on wide treatment fields, were associated with a substantial cardiac mortality increase. Fortunately, breast irradiation has benefited from recent technological progresses allowing significant cardiac sparing, such as intensity-modulated radiation therapy (IMRT) [5], proton therapy [6] or deep-inspiration breath hold (DIBH)[7].

CONCLUSIONS: Awareness that heart disease is the LCOD among women declined from 2009 to 2019, particularly among Hispanic and non-Hispanic Black women and in younger women (in whom primordial/primary prevention may be most effective). Adjuvant radiotherapy following breast-conserving surgery decreases local recurrences and improves survival outcomes in breast cancer [1,2]. However, radiotherapy is associated with an increased risk of cardiovascular complications as a result of cardiac irradiation, including ischemic heart disease [3-5], valvulopathy [3,6-9], congestive heart failure [3,5], myocarditis [9], arrhythmia [3] and myocardial infarction [5,10]. A case control study has demonstrated an increased risk of major coronary events by 7.4% for every Gy increase in mean dose to the heart [11].
