A Large Abdominal Mass in a Child Presenting with Fever and Urinary Symptoms: An Infected Urachal Cyst

A 6-year-old boy was admitted to a tertiary pediatric hospital with a two-week history of fevers, night sweats, lower abdominal pain, and dysuria. Initial inflammatory markers were elevated, but urine culture was negative for urinary tract infection (UTI). Abdominal ultrasound revealed a complex hypoechoic mass lesion arising from the anterior abdominal wall, extending from the umbilicus into the pelvis and compressing the bladder, initially concerning for possible soft tissue malignancy (Figure 1).
