AbstractIn this review, we consider the diverse risk factors in diabetes patients beyond hyperglycemia that are being recognized as contributors to diabetic peripheral neuropathy (DPN). Interest in such alternative mechanisms has been encouraged by the recognition that neuropathy occurs in subjects with metabolic syndrome and pre-diabetes and by the reporting of several large clinical studies that failed to show reduced prevalence of neuropathy after intensive glucose control in patients with type 2 diabetes. Peripheral neuropathy is one of the most common neurologic conditions of late adulthood. Over 5% of those older than 55 years of age and over 30% of those older than 80 have probable or definite neuropathy.1 Neuropathy is a major cause of morbidity and reduced quality of life (QOL) due to pain, gait disturbance, foot ulceration, and amputation, particularly among diabetic patients. Up to half of patients with neuropathy have diabetes, and about 40% have idiopathic neuropathy. A number of studies suggest that prediabetes and metabolic syndrome are risk factors for idiopathic neuropathy,2-4 although there are conflicting data.

This study aimed to compare the effect of high-intensity interval training (HIIT) with moderate-intensity continuous training (MCT) on endothelial function, oxidative stress and clinical fitness in patients with type 1 diabetes. Thirty-six type 1 diabetic patients (mean age 23.5 ± 6 years) were randomized into 3 groups: HIIT, MCT, and a non-exercising group (CON).
