The clinical manifestations vary greatly among individuals and easy to be misdiagnosed. Once misdiagnosed, sudden malignant hypertension, heart failure or cerebral hemorrhage cause death in severe patients. In this case report, we describe a 47-year-old female patient with pheochromocytoma misdiagnosed as a stromal tumor who developed sudden cardiac arrest when pneumoperitoneum was established after anesthesia and then developed acute catecholamine cardiomyopathy. This case served as a reminder to pay more attention to the possibility of misdiagnosis and adequate preoperative anesthetic preparation in patients with asymptomatic pheochromocytoma.
