Sudden Cardiac Death Risk-Stratification in Ventricular Preexcitation: A Tale of Two Eras

Primary prevention of sudden cardiac death (SCD) is an issue well known to electrophysiologists. In asymptomatic patients with ventricular preexcitation (VPE), where the estimated annual SCD risk may be as low as 1 in a 1000, risk assessment may be particularly difficult (1). However, it is axiomatic that you cannot make an asymptomatic individual feel better, but you cannot develop a therapeutic plan for someone who has died. Such is the conundrum of risk stratification.
