A Mysterious Right Atrial Jet

A 60-YEAR- OLD woman, with a history of chronic obstructive pulmonary disease and cachexia, presented with an ST-elevation myocardial infarction and new heart failure. Transthoracic echocardiography showed an ejection fraction of 25%, normal right ventricular size and function, and no significant valvular abnormalities. Angiography revealed severe disease of her left circumflex and left anterior descending coronary arteries. The patient was taken to the operating room for coronary artery bypass graft surgery.
