Orofacial dystonia and associated bulbar symptoms in multiple system atrophy: A blinded video analysis

We read with interest the article by Tongchuam and colleagues [1] on orofacial dystonia and associated bulbar symptoms in multiple system atrophy (MSA). Dystonia is an important but under-investigated symptom that may be present in both idiopathic Parkinson's disease and atypical parkinsonian conditions, including MSA [2]. Cranial dystonia, and in particular orofacial dystonia, is a relatively common manifestation in MSA, consisting of tonic spasms of the lower face and lips, and conveying an expression that resembles the so-called “tetanic risus sardonicus” [3].
