The impact of advance care planning (ACP) interventions is typically measured by completion of advance directives (AD), but this approach may miss other aspects of engagement. Surveys can measure progress in readiness to engage that falls short of document completion as well as other domains such as communication with clinical providers and designated medical decision makers. Longer surveys may capture nuances in engagement but are often infeasible in research with busy patients and providers.
