A 66-year-old man underwent a successful heart transplant (HTx) in 2005. He tested anti –hepatitis C virus (HCV) positive before HTx; HCV RNA was not determined. In the post-operative period, the patient experienced acute rejection and kidney insufficiency, which improved after treatment. In 2009, he was diagnosed with HCV infection, genotype 1b, and high viral load. A liver biopsy d emonstrated chronic active portal hepatitis, fibrosis, and cholangiolar proliferation. Biliary complications were ruled out.
