Middle-Aged Man with a Leg Rash: Exercise-Induced Vasculitis

A 40-year-old man with no significant medical history presented to the Emergency Department (ED) with an erythematous, petechial rash localized to the bilateral lower extremities over the past several hours (Figures  1 and 2). The patient had been seen at an Urgent Care and transferred to the ED with a recommendation to begin intravenous antibiotics and to admit the patient for a suspected diagnosis of bilateral lower extremity cellulitis. The patient reported some associated mild pruritus. He denied the use o f any new soaps, laundry detergents, lotions, medications, or any potential exposures.
