Deep Venous Thrombosis and Pulmonary Embolism After Minimally Invasive Transforaminal Lumbar Interbody Fusion: Report of 2 Cases in 315 Procedures. Orthop Clin North Am. 2020 Jul;51(3):423-425 Authors: Olinger CR, Gardocki RJ Abstract When the guidelines of the North American Spine Society concerning deep venous thrombosis (DVT) prophylaxis were followed, only 2 (0.63%) of 315 patients with minimally invasive transforaminal lumbar interbody fusions developed DVT complications over a 9-year period. Based on these findings, mechanical DVT prophylaxis appears to be adequate in patients undergoing elective spinal surgery, with no current support for pharmacologic prophylaxis. CONCLUSIONS: Overall adherence to published guidelines for VTE prophylaxis has improved compared with other published reviews on the topic, but documentation of patients' VTE risk in files is poor. Overuse in low-risk patients may be an unintended consequence of the widespread advocacy of LMWH use in hospital, highlighting the importance of adequate VTE risk stratification. A 12-year-old female with SARS-CoV-2 infection presented as phlegmasia cerulea dolens with venous gangrene.. Emergent mechanical thrombectomy was complicated by a massive pulmonary embolism and cardiac arrest for which extracorporeal cardiopulmonary resuscitation and therapeutic hypothermia were employed.
