Clinical shortcuts in the differential diagnosis of pruritus.

This article summarizes some clinical shortcuts. Simple procedures such as taking a detailed medical history can provide clues to the underlying cause of chronic pruritus in order to achieve targeted diagnostic workup or to avoid unnecessary testing. If clinical shortcuts are not identified, we recommend a structured medical history, which is also discussed in this article. PMID: 32488462 [PubMed - as supplied by publisher]

As a primary care physician at an academic community health care system in Massachusetts, I received a rapid introduction to telehealth this year. Within days after Massachusetts declared a state of emergency in response to the spread of COVID-19, almost all of our patient visits became telemedicine visits. Our staff reached out to patients to inform them of different ways they could get in touch with their doctor. Many would be able to gain access to health care through a health app connected to their healthcare web portal, or through a phone call or video call. The enormous potential of telehealth was apparent to me with...

Conclusion: IL-31 is the key mediator for pruritus in both AD and Ps patients. IL-4/31 axis and IL-33/13 axis play distinct roles in the pathogenesis of Atopic dermatitis and Psoriasis. Interleukin serum levels were not correlated with itch and disease severity in both conditions. PMID: 32606880 [PubMed]

Authors: Abstract Atopic dermatitis (AD), a long-lasting skin disease characterized by intense itching and dry, red, swollen and cracked skin, affects between 2% and 10% of Japanese adults. In Japan, patients with moderate-to-severe or very severe AD have limited treatment options, most with unwanted side effects. There is a need for therapies that provide safe and effective long-term control. The underlying mechanism of AD may differ between Asian and Western patients, and this study aimed to evaluate dupilumab, a drug that blocks key molecules that cause AD, in Japanese patients with moderate-to-severe AD. The ...

Abstract The roles of serotonin and noradrenaline in the modulation of chronic pruriceptive processing currently remain unclear. To clarify the contribution of serotonin and noradrenaline to chronic itch, the effects of the administration of antidepressants or noradrenaline reuptake inhibitors were evaluated in the present study. A pretreatment with milnacipran, a serotonin and noradrenaline reuptake inhibitor, and mirtazapine, a noradrenergic and specific serotonergic antidepressant, attenuated the induction of spontaneous scratching behavior in mice with chronic itch. The administration of a serotonin reuptake i...
