Talus Osteomyelitis: An Uncommon Manifestation of Melioidosis

The patient was a 56-year-old man with progressive right ankle pain and swelling with no fever or systemic symptoms. He had poorly controlled diabetes mellitus and a remote history of splenic melioidosis 14 years ago. Initial radiographs were unremarkable (Fig. 1A). MRI of the right ankle showed marrow edema and enhancement of the talus (Fig. 1B –D) suspicious for osteomyelitis versus bone marrow edema syndrome. He underwent an open biopsy of the talus and tissue cultures grew Burkholderia pseudomallei.
