CONCLUSIONS: Laterality judgment impairment was shown in CPRS1, upper limb pain, hand and wrist pain, carpal-tunnel syndrome, facial pain, knee osteoarthritis, and leg pain. No conclusions could be drawn in low back pain, due to the low-quality evidence and differing results. ConclusionFurther efforts are warranted to adapt neuromodulation/neurostimulation protocols per patient owing to neuroplastic changes that favor vs. hinder improvement of function. A sole solution for all cases is not suggested. An individualized approach relying on biomarkers and clinical outcomes should ease the implementation in clinical practice of noninvasive neuromodulation as an innovative therapeutic option to speed-up recovery towards gains beyond those already reached, thus to further improve the quality of life.

Ectopic impulse discharge (ectopia) generated in the soma of afferent neurons in dorsal root ganglia (DRG) after nerve injury is believed to be a major contributor to neuropathic pain. Spinal cord stimulator (SCS) implantation is an effective tool for treatment of various conditions that include complex regional pain syndrome, peripheral vascular disease, neuropathic pain, refractory angina, and failed back surgery syndrome (FBSS). SCSs are placed in the thoracic region to treat low back pain and lower extremity radicular pain. Permanent SCS is indicated for chronic nonmalignant pain of neuropathic origin with a positive trial. [1] Complications of SCS have been reported at 30%-40%, with hardware related problems, as in lead failure and migration, more common than infection.
