On behalf of all the coauthors of this study we appreciate the Editorial Comments provided. In our tertiary care practice we commonly see patients referred to us after failed percutaneous nephrolithotomy (PCNL), when failure was due to the inability to secure a wire down the ureter. In some cases these failures are the result of an obstructing stone at the ureteropelvic junction or a large renal pelvis with a narrow ureteropelvic junction. In other cases the stones are impacted in a pole of the kidney, there is infundibular stenosis, or a true diverticulum.
