A Large Isthmal Diverticulum Mimicking Hydrosalpinx

A 28-year-old nulliparous woman presented to the hospital with a history of secondary infertility for 2 years. The infertility screening examinations were all normal excepted for a right “hydrosalpinx” identified via transvaginal sonography and hysterosalpingography (Figure 1). During a diagnostic laparoscopy, a large diverticulum sized 4-cm-by-3-cm was found at the isthmus of right fallopian tube after hydrotubation with diluted methylene blue (Figure 2). Both ends of isthmus w ere found opening into the diverticulum when the diverticular wall was incised (Figure 3).
