Commentary to: “Eighteen years of experience with pediatric nutcracker syndrome: The importance of conservative approach”

Authors present a comprehensive long-term follow-up of patients diagnosed with Nutcracker syndrome (1). As it is discussed in their article, usually the Nutcracker syndrome should be treated conservatively considering how challenging the surgical options can be. However, in cases in which patients become symptomatic with persisting hematuria, dyspareunia, dysuria or dysmenorrhea due to ovarian plexus congestion or varicocele, surgical treatment should be offered(2). Nonetheless, current surgical options are technically demanding and very invasive with significant morbidity.
