Role of the sympathetic nervous system in pain

Abstract: The involvement of the sympathetic nervous system in chronic pain conditions has been well described and recognized for over a century. However, the exact mechanism of the relationship has not been fully explained. In certain chronic pain conditions (e.g. complex regional pain syndrome (CRPS)), the presence of sympathetic signs forms part of the diagnostic criteria.Typical management will involve a multidisciplinary approach, including physical, pharmacological, and psychological therapies. Interventional techniques such as sympathetic blocks can also be used; however evidence of efficacy for many treatments remains poor.

CONCLUSIONS: Relative motion flexion splinting affords early active motion and hand use with excellent range of motion achieved following acute open boutonniere repair or closed boutonniere rupture with less morbidity than conventional management. Chronic boutonniere deformity will respond to relative motion flexion splinting if serial casting can place the proximal interphalangeal joint in less than -20° extension, and the patient actively uses the hand in a relative motion flexion orthosis for 3 months, recovering flexion. CONCLUSION: This study showed that calcitonin could be helpful analgesic agent in different painful situations. Calcitonin can be considered an eligible treatment for acute pains related to vertebral fractures and a feasible alternative for the treatment of the acute and chronic neuropathic pains where other medications might fail. We report PMPS in a 39-year-old patient after revision operation of breast reduction surgery. Postoperatively, the patient showed symptoms of severe persistent pain, swelling, cutaneous temperature changes, and hyperhidrosis in the affected breast. Subcutaneous infiltration with triamcinolone acetonide and prilocaine only relieved the symptoms temporarily. Botox injections did not relieve her symptoms of sweating either.
