Patterns of splenic arterial enhancement on computed tomography scan are related to portal venous hypertension

Objectives: We have previously shown that patterns of splenic arterial enhancement on computed tomography scan change following liver transplantation. We suggested that this is related to changes in portal venous pressure. The aim of this study was to see if similar patterns occur in patients with and without portal hypertension and in patients before and after portal systemic shunts (transjugular portosystemic shunts). Methods: We evaluated contrast enhanced computed tomography scans in patients being evaluated for liver disease and compared those from patients with and without portal hypertension. In addition we evaluated patients who had computed tomography scans before and after transjugular portosystemic shunts shunts. Splenic arterial enhancement was evaluated using Hounsfield units (pixel counts). Results: Twenty-four patients with clinically significant portal hypertension were compared to 91 without. Refractory variceal bleeding and early re-bleeding are associated with high mortality in patients with chronic liver disease (CLD) complicated by portal hypertension. A fully-covered self-expanding metal stent (SEMS) (SX-ELLA Danis stent; ELLA-CS, Hradec Kralove, Czech Republic) has been reported to have excellent rates of technical success and initial bleeding control; however, studies to date are small and limited to Europe and Asia. Recently, we showed the feasibility and safety of a simple novel technique for Endoscopic ultrasound (EUS) guided PPG measurement (PPGM) in a pilot study. EUS-guided liver biopsy (EUS-LB) has been shown to be a safe and effective alternative to percutaneously or Interventional Radiology performed liver biopsy for the diagnosis of liver disease. We aimed to assess feasibility and safety of concomitant EUS-guided PPGM with EUS-LB in a single session.

Gastric varices (GV) and Portal Hypertensive Gastropathy (PHG) are well recognized complications of portal hypertension and carry a risk of gastrointestinal bleeding. Percutaneous endoscopic gastrostomy (PEG) tubes are a valuable tool for long term nutrition, among other indications, and are often indicated in patients with liver disease and portal hypertension. However, the safety and outcomes of PEG tube placement in patients with pre-existing PHG and GV is not well established in current literature.

ConclusionsPatients who receive neoadjuvant chemotherapy for liver metastasis surgery have a greater splenic volume increase, which correlates with NAFLD and a lower survival.

ConclusionsSome of the methods for the estimation of cardiovascular risk (ACC/AHA ASCVD 2013, new model 2 2018, and, also Framingham score 2008) have a clear advantage over Framingham score 2004 and “Progetto Cuore” of the Italian Institute of Health in terms of correlation with the severity of NAFLD. They are, therefore, more clinically useful.
