IgE-mediated food allergy (FA) accounts for the leading cause of anaphylaxis in children, though fatal reactions remain rare.1,2,3 Strict adherence to avoidance diets and use of FA emergency management plans can decrease reaction rates significantly, with one study demonstrating a 60-fold reduced frequency of severe FA reactions.4 However, which patients experience severe vs mild symptoms, as well as contributory factors, remains an area of active study.5,6 Currently, no unified definition of a severe FA reaction exists, calling into question which symptom or compilation of symptoms during a reaction constitute a severe label.

CONCLUSIONS: Both animal models showed comparable results on the residual allergenicity of partial hydrolyzed whey proteins, but none of the centers was able to differentiate between the residual sensitizing capacities of the pWH-B and -C based on a single elicitation parameter in the murine model. Differences in microbiota composition might contribute to the robustness of the food allergy murine model. For a well-balanced prediction on the potential allergenicity of hydrolyzed infant formulas a multiple murine parameter model is suggested to decrease the risk of false positive or false negative results. We thank Grigoletto et al for their thoughtful letter regarding our report.1 We agree with the authors ’ conclusion that clinicians should consider history of asthma and asthma control status when managing children with anaphylaxis. In support of their conclusions, the authors cite a study evaluating anaphylactic reactions from peanuts and tree nuts in an outpatient allergy center.2 In contrast to our report, this study includes limited allergens, and may represent a more severe cohort of patients with a higher predilection for asthma and/or severe anaphylaxis based on their referral to an allergy center.

CONCLUSION: It is essential that implementation of focused training programs about anaphylaxis symptoms and signs as well as practical instructions of when and how to use an epinephrine auto-injector will decrease preventable morbidities and mortalities due to bee-stings in this selected high-risk population of beekeepers and their family members, as well as other fieldworkers under risk.
