Cutaneous Lupus Erythematosus: Progress and Challenges

AbstractPurpose of ReviewThe review provides an update on the diagnosis, pathogenesis, and treatment of cutaneous lupus erythematosus (CLE).Recent FindingsDiagnostic challenges exist in better defining CLE as an independent disease distinct from systemic lupus erythematosus with cutaneous features and further classifying CLE based on clinical, histological, and laboratory features. Recent mechanistic studies revealed more genetic variations, environmental triggers, and immunologic dysfunctions that are associated with CLE. Drug induction specifically has emerged as one of the most important triggers for CLE. Treatment options include topical agents and systemic therapies, including newer biologics such as belimumab, rituximab, ustekinumab, anifrolumab, and BIIB059 that have shown good clinical efficacy in trials.SummaryCLE is a group of complex and heterogenous diseases. Future studies are warranted to better define CLE within the spectrum of lupus erythematosus. Better insight into the pathogenesis of CLE could facilitate the design of more targeted therapies.

In conclusion, inflammatory cytokines such as IL-6 involved in inflammation and differentiation of Th17 cells are probably responsible in part for Th17 activity in CLE. IL-17, IL-23, and IL-6/IL-6R (IL-6 receptor) inhibitors may be good treatments for CLE patients. Patients with rheumatologic illness are at increased risk for infection by virtue of their immune dysregulation as well as use of immunosuppressive medications. Along with disease flares, infection is the leading cause of hospital admission in patients with rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE)1. Complicating the care of these patients is the rate of patient-reported penicillin allergy, which has ranged 20-28% in the outpatient setting2-4, compared to the general population where the reported rate is ∼10% in the outpatient setting5, and ∼ 20% in the inpatient setting6.

Conclusions The long-term risks of autoimmune disorders are significantly higher in patients with allergic diseases. Allergic diseases and autoimmune disorders show age- and sex-related clustering patterns.

Hydroxychloroquine (HCQ, Plaquenil ®) is an antimalarial agent with immunosuppressive and cytotoxic properties that is considered safe and effective in pregnant patients with a history of mild lupus nephritis (LN).1-2 Hypersensitivity to hydroxychloroquine usually manifests as maculopapular exanthems within two to 33 days and rarely as eosinophilia or other systemic symptoms.3 Several prolonged desensitization protocols, ranging from four to 36 days in duration, have been proposed to manage prior histories of HCQ hypersensitivity.
