New and Established Therapeutic Options for the Treatment of Systemic Lupus Erythematosus.

[New and Established Therapeutic Options for the Treatment of Systemic Lupus Erythematosus]. Praxis (Bern 1994). 2020;109(5):341-345 Authors: Goehner Schweizer K, Maurer B Abstract New and Established Therapeutic Options for the Treatment of Systemic Lupus Erythematosus Abstract. Systemic lupus erythematosus is a multisystem autoimmune disease. The mainstay of therapy are antimalarials, mostly hydroxychloroquine, which should be given whenever possible. Belimumab is in the meantime a well-established add-on treatment. In the first place, joint manifestations as well as skin and fatigue improve. It was also shown to reduce severe flares and steroid use. Another therapeutic option in the future might be the interferon anifrolumab due to positive results in new studies. In case of renal manifestations mycophenolate mofetil is widely used. PMID: 32233759 [PubMed - in process]

Authors: Wu H, Chang C, Lu Q Abstract Systemic lupus erythematosus (SLE) is a life-threatening autoimmune disease that is characterized by dysregulated dendritic cells, T and B cells, and abundant autoantibodies. The pathogenesis of lupus remains unclear. However, increasing evidence has shown that environment factors, genetic susceptibilities, and epigenetic regulation contribute to abnormalities in the immune system. In the past decades, several risk gene loci have been identified, such as MHC and C1q. However, genetics cannot explain the high discordance of lupus incidence in homozygous twins. Environmental fact...

Authors: Ceribelli A, Selmi C Abstract Genomic predisposition fails to fully explain the onset of complex diseases, which is well illustrated by the largely incomplete concordance among monozygotic twins. Epigenetic mechanisms, including DNA methylation, chromatin remodeling, and non-coding RNA, are the link between environmental stimuli and disease onset on a permissive genetic background in autoimmune and chronic inflammatory diseases. Autoimmune diseases now include almost 100 conditions and are estimated to cumulatively affect up to 5% of the world population with a healthcare expenditure superior to cancer wor...

Authors: Pablo AH, Breaud AR, Clarke W Abstract Immunosuppressant medications help suppress the immune system response through inhibition of various checkpoints in the regulatory biochemical pathway. This is useful in prevention of organ rejection in transplantation or in the treatment of autoimmune diseases such as lupus or rheumatoid arthritis. Quantification of immunosuppressive drugs in blood is needed clinically for optimization of treatment and to avoid toxicity or unwanted side effects. Here, we describe a quantitative method to determine the concentration of cyclosprine A, tacrolimus, sirolimus, and everoli...

In the largest observational study thus far investigating the drug hydroxychloroquine as a treatment for COVID-19, researchers found little evidence that it helps, and worrying evidence that the medication may cause harm. In a study published May 22 in the journal Lancet, scientists in the U.S. and Switzerland report on an analysis of more than 96,000 people hospitalized with confirmed COVID-19 in 671 hospitals on six continents. Nearly 15,000 patients were treated with one of the following: chloroquine (which is an older version of hydroxychloroquine), hydroxychloroquine, or either of those drugs in combination with an an...

Dysregulated cytokine expression by T cells plays a pivotal role in the pathogenesis of autoimmune diseases. However, the identification of the corresponding pathogenic subpopulations is a challenge, since a distinction between physiological variation and a new quality in the expression of protein markers requires combinatorial evaluation. Here, we were able to identify a super-functional follicular helper T cell (Tfh)-like subpopulation in lupus-prone NZBxW mice with our binning approach "pattern recognition of immune cells (PRI)". PRI uncovered a subpopulation of IL-21+ IFN-ghigh PD-1low CD40Lhigh CXCR5-Bcl-6-T...

Autoimmune diseases comprise a spectrum of illnesses and are on the rise worldwide. Although anti-nuclear antibodies (ANA) are detected in many autoimmune diseases, up to 20% of healthy women are ANA+ and most will never develop clinical symptoms. Further, disease transition is higher among ANA+ African Americans compared to European Americans.

This study characterized trends in utilization of CABG in patients with CTD and the impact of the disease on mortality, in-hospital complications, length of stay (LOS) and costs. METHODS: The 2005-2015 National Inpatient Sample was used to identify all adult patients undergoing isolated CABG. The CTD cohort included rheumatoid arthritis, lupus erythematosus, antiphospholipid syndrome (APLS), among others. Hierarchical multivariable logistic models were used to calculate the independent impact of CTD on clinical outcomes and costs. RESULTS: Of an estimated 2,101,591 million patients, 41,567 (1.8%) were diagnosed w...

AbstractHydroxychloroquine (HCQ) is an old medication for malaria. In addition to handling this parasitic disease, HCQ is also used to treat a number of autoimmune disorders including rheumatoid arthritis and systemic lupus erythematosus when other medications are not effective. Recently a new viral infection (COVID-19) is rocking the entire world so much that it has already taken more than 200,000 lives throughout the world within the last two months and the World Health Organization was forced to declare it as a pandemic on March 11, 2020. Interestingly, some reports indicate that this wonder drug may be also beneficial ...

AbstractSystemic lupus erythematosus is a multisystem autoimmune disease. Apart from usual treatments, approximately 50% of lupus patients use complementary medicine. Resveratrol is a phytoalexin with various pharmacological properties. We hypothesised that prophylactic treatment with resveratrol may abrogate manifestations in pristane-induced murine model of lupus-like disease and piperine; a bio-enhancer of resveratrol may enhance these properties. The prophylactic effect of resveratrol (25  mg/kg body weight: P-Res) alone and in combination with piperine (2.5 mg/kg body weight: P-RP) were assessed. P-Res and P...
