Necessary Components of Psychological Treatment for Chronic Pain: More Packages for Groups or Process ‐Based Therapy for Individuals?

AbstractThis journal recently published a paper by Sharpe et al., entitled “Necessary components of psychological treatments in chronic pain management programs: A Delphi study” (Sharpe, Jones, Ashton‐James, Nicholas,&Refshauge, 2020). These researcher sought the views of authors of relevant RCTs published up to September 2016 and supplemented these with input from psychologists working in pain management programs in Australia.

AbstractThe use of cocaine constitutes a major health problem. Cocaine use is associated with acute and chronic complications that might involve any system, the most common being the cardiovascular system. The precise incidence of cocaine-induced cardiomyopathy remains mysterious and probably underreported. Cocaine use should be considered in young patients presenting with chest pain or heart failure without other underlying risk factors. Cocaine-related cardiovascular complications can be acute or chronic and include ischemic and non-ischemic events. Frequent cocaine users have a seven-fold higher risk of myocardial infar...

In this study, we present three cases of testicular infarction and discuss their clinical characteristics, imaging features and clinical management. PATIENTS AND METHODS: Three adult males with prolonged epididymitis presented with chronic unilateral testicular pain, tenderness, and palpable swelling, including left varicocele in one case and hydrocele in the other two cases. Patient's symptoms were not relieved after antibiotic therapy. We analyzed the diagnosis, management, and outcome of these three cases of testicular infarction resulting from prolonged epididymitis. This includes the clinical characteristics, feat...

Functional abdominal pain is commonly related to visceral hypersensitivity (VH) and recent studies have shown success in management of such patients with Amitriptyline. A subgroup of patients with chronic RUQ pain post cholecystectomy routinely fail medical management and are referred for evaluation of Sphincter of Oddi (SOD) dysfunction/papillary stenosis. The association of RUQ pain, dilated common bile duct (CBD) and elevated liver function tests (LFT ’s) is strongly suggestive of papillary stenosis as it has been shown to benefit the most from biliary sphincterotomy.

Over the past decade, increased awareness by the medical profession of the devastating consequences of opioid addiction has resulted in substantial efforts to limit the number of opioid prescriptions for both perioperative pain management and chronic pain. While these efforts have had some success, opioid misuse remains a crisis, which we in the orthopaedic community have a particular opportunity to address. It is the belief of the undersigned that progress depends on improved research methods and reporting to further the understanding of pain experience and response to management, with the end goal of identifying more eff...

The endoscopic ultrasound (EUS) fine needle aspiration (FNA) technique has been refined improving tissue acquisition and allowing targeted analgesic injection. During the last years, new research and guidelines advocate for conservative management of low risk pancreatic cysts. Simultaneously, an increasing number of reports have shown the limitations of celiac plexus block and neurolysis to treat chronic abdominal pain. We aims to describe changes in EUS practice over a decade.

Jeffrey A. SingerPublic health interventions entail non ‐economic as well as economic trade‐offs. Some trade ‐offs can involve other aspects of public health.I havewrittenabout how blanket bans on elective medical procedures combine with the fear already infused in the public to cause crucial delays in necessary health care. This adds to human suffering from causes other than the COVID-19 virus. Many people with chronic conditions, particularlychronic pain patients, are disproportionately affected by reduced access to routine care. Then there ’s the dramatic drop ‐...

CONCLUSION: On the basis of a broadly based research and participatory development process, a set of indicators has been developed that enables comprehensive evaluation of the regional quality of care of cross-indication, integrated care models focusing on common diseases. In order to be able to increasingly evaluate aspects of care coordination and patient orientation, health promotion as well as nursing, palliative and emergency care in the future, it would be helpful if routine data were collected or made accessible in these areas as well.
