Conclusions: Clinical symptoms and spinal fusion efficiency were not significantly different between CBT and PS except for postoperative improvement in low back pain. We report a case of double-level lumbar spondylolysis at L4 and L5 that was successfully treated with the double "smiley face" rod method. A healthy 29-year-old man who presented with a 6-year history of chronic low back pain was referred to us for surgical treatment. A 58-year-old woman presented to our physiatric spine clinic with low back pain after falling backwards from an unsecured seat at a movie theater 3  weeks prior. She presented to the local emergency department where computed tomography (CT) imaging of the lumbar and cervical spine were negative for fractures. Magnetic resonance imaging of the lumbar spine revealed mild degenerative changes in the lower lumbar spine. She denied significant medi cal or surgical history. She also denied systemic symptoms or the use of any anticoagulation medications.
