Evaluation of p53 and Its Target Gene Expression as Potential Biomarkers of Cholangiocarcinoma in Thai Patients.

CONCLUSION: Our results demonstrated statistically significant association between down-regulation of WIP1 with non-papillary type and with high p53 protein level, and PAI-1 was down-regulated in almost all CCA. Therefore, expression level of WIP1 and PAI-1 may be useful for predicting p53 functional status and as a potential diagnostic marker of CCA, respectively. PMID: 32212809 [PubMed - in process]

Cholangiocarcioma (CCA) is a cancer with extremely poor survival, with 5-year survival as low as 5-10% in the published literature, and a median survival of 3 to 6 months for unresectable disease. While relief of biliary obstruction with endoscopic (plastic or metal stenting) or percutaneous drainage have been the mainstay of palliative therapies, these techniques are limited in their life-prolonging results. Photodynamic therapy (PDT) and radiofrequency ablation (RFA) are newer endoscopic technologies that allow for palliation with good safety profiles and improved survival compared with stent placement alone.

Bile duct cancer is a diagnostic challenge due to difficulty in obtaining tissue via ERCP bile duct brushings (BDB) coupled with a high threshold for a malignant diagnosis on cytology. Recent investigations from Massachusetts General Hospital have shown the value of next generation sequencing (NGS) as an ancillary test in the diagnosis of indeterminate strictures. Here, we present the role of NGS and FISH (fluorescence in-situ hybridization) in determining risk of malignancy one year from index ERCP in a cohort of patients with negative cytology.

Conclusion: The diagnostic efficiency for EHCC of MUC3A is obviously superior to CA19-9 and CEA, and a high level of serum MUC3A indicates a poor prognosis, therefore, MUC3A can be used as a potential diagnostic and prognostic biomarker for EHCC.
