Peritoneal dialysate tamponading a massive retroperitoneal hemorrhage

A 47-year-old man with end-stage renal disease on peritoneal dialysis (PD) underwent a total hip arthroplasty for a left femoral fracture. His comorbidities were notable for mechanical mitral and aortic valves, for which he took warfarin with a goal international normalized ratio of 2.5 –3.5. Postoperatively he was continued on his home PD regimen, and his warfarin was restarted after having been held during the perioperative period. On postoperative day 17, he became unresponsive during PD drainage, with a blood pressure of 62/34 mm Hg.
