Transtemporal Suprajugular Approach with Neck Dissection for Jugular Foramen Tumor Resection: Operative Video

J Neurol Surg B DOI: 10.1055/s-0040-1701680This video demonstrates the transmastoid suprajugular approach with neck dissection to a solitary fibrous tumor involving the jugular foramen and upper cervical region. This patient was a 39-year-old man who presented with dysphagia and cranial nerve (CN) XI and CN XII palsies. Imaging revealed a large homogenously enhancing lesion involving the jugular foramen and extending into the retropharyngeal space (Fig. 1). Radiographic findings supported a diagnosis of jugular foramen schwannoma. After an initial period of observation, the tumor demonstrated significant growth, and the patient agreed to proceed with surgery. The suprajugular approach allowed for exposure and resection of the tumor without mobilization of the facial nerve. The patient had an excellent clinical outcome with House–Brackmann grade-1 facial function, safely tolerated a regular diet, had intact CN XI function, and had a stable CN XII palsy (Fig. 2). CONCLUSIONS: In cases of severe infection with considerable bony destruction, insertion of an aPMMA strut graft is a novel technique that should be considered in order to provide strong anterior-column support while directly delivering antibiotics to the infection bed. While the active infection is being treated medically, this structural aPMMA support bridges the time it takes for the patient to be converted from a catabolic to an anabolic state, when it is ultimately safe to perform a definitive, curative fusion surgery. Conclusions: For one-level ACDF, the anchored cage with screws has some advantage compared to the stand-alone PEEK cage in avoiding dysphagia and restoring disc space height and cervical lordosis. We suggest the anchored spacer may be a good substitute for the stand-alone cage in one-level ACDF. Level of Evidence: Level II

Conclusion. An early self-directed HEP program was acceptable to patients and has the potential to be safely administered to patients immediately after ACDF. Benefits were noted for short-term neck pain and long-term opioid utilization.
