Bone Disease in Patients with Ehlers –Danlos Syndromes

AbstractPurpose of ReviewTo summarize the bone findings, mainly bone mass and fracture risk, in Ehlers –Danlos syndromes (EDS).Recent FindingsLow bone mineral density and fractures seem to be frequent in some of the rare EDS types (kyphoscoliotic, arthrochalasia, spondylodysplastic, and classic-like EDS). For the more prevalent hypermobile and classic EDS types, some case –control studies found mildly decreased bone mineral density, but it was not clear that fracture rates were increased. Nevertheless, abnormalities in vertebral shape seem to be common in classical and hypermobile EDS types. In a cohort of individuals with EDS followed since birth, no fractures wer e observed during infancy.SummaryBone mineral density varies widely among the different types of EDS, and vertebral abnormalities seem to be common in classical and hypermobile EDS. It might be justified to perform spine radiographs and bone mineral density assessments in newly diagnosed EDS.

ConclusionsRadiographic vertebral fractures were common among Thai postmenopausal women (~ 29%). These findings suggest that approximately one in three postmenopausal women has undiagnosed vertebral fracture. Radiographic diagnosis should therefore be an essential investigation for identifying and confirming the presence of vertebral fractures.

Conclusions: Open reduction of displaced femoral neck fractures in nonelderly adults is associated with a greater hazard of reoperation without significantly improving reduction. Prospective randomized trials are indicated to confirm a causative effect of open versus closed reduction on outcomes after femoral neck fracture. Level of Evidence: Therapeutic Level III. Conclusion. Age>70 years, osteoporosis, and PJA greater 0° were identified as significant risk factors for PJF. Therefore, the TLJ level can be considered as UIV selectively for patients younger than 70 years without osteoporosis and with lordotic preoperative PJA. Level of Evidence: 3

In this study, we compared the treatment outcomes of pins and plaster with locking plates in elderly patients over 65 yr of age. A 44-year-old woman with a history of hypertension and partial gastrectomy presented to the emergency department with low thoracic back pain of 2  days' duration without red flag symptoms. The physical examination was significant for point tenderness to palpation in the lower thoracic spine without overlying skin changes or neurologic symptoms. Considering her large body habitus and history of gastrectomy, a thoracic radiograph was obtained to rule out compression fracture due to osteoporosis (Figure 1).

CONCLUSIONS: A high index of suspicion is needed to diagnose sacral insufficiency fracture after LS arthrodesis. A trial of conservative management is reasonable for select patients; potential surgical indications include refractory pain, neurological deficit, fracture nonunion with anterolisthesis or kyphotic angulation, L5-S1 pseudarthrosis, and spinopelvic malalignment. Lumbopelvic fixation with iliac screws may be effective salvage treatment to allow fracture healing and symptom improvement.
