10: Preoperative levator ani muscle and pudendal nerve injections for pain control after vaginal reconstructive surgery: A three-arm randomized controlled trial

To test the hypothesis that preoperative levator ani muscle (LAM) and transvaginal pudendal nerve (PN) injections with bupivacaine and dexamethasone would improve postoperative pain after vaginal apical prolapse repair.

Transverse Vaginal Septums (TVS) are congenital, obstructive anomalies of the female genital tract typically presenting with abdominal pain and amenorrhea around menarche. Currently, the literature surrounding surgical management is scarce and poorly described. As well, both early and long term postoperative complications are common owing to the complexity of the surgical repair. Lastly, often these patients require intensive vaginal dilation schedules postoperatively, the details and outcomes of which are poorly described.

CONCLUSIONS: Based on our experience, IIA embolization does cause buttock claudication of a certain degree. However, the most of them experienced mild discomfort rather than such symptoms severely affect their quality of life. Considering the risks of general anesthesia and complications of surgical procedures, IIA reconstruction along with EVAR may not be necessary. PMID: 32214071 [PubMed - as supplied by publisher]

Authors: Consalvo V, D'Auria F, Salsano V Abstract BACKGROUND: Chronic pain still occurs in 10-12% of patients who undergo surgical groin hernia repair. Considering the high prevalence of this pathology, we performed a single-center prospective study comparing the laparoscopic trans-abdominal pre-peritoneal (TAPP) approach to the standard surgical open technique for primary uncomplicated hernia repair. METHODS: A prospective cohort of 278 patients was extracted from our dataset: 121 received a laparoscopic TAPP approach, and 157 were treated by the Lichtenstein technique in case of inguinal hernia or by the dep...

Conclusions We present a simple arthroscopic technique using a single suture anchor placed in the ulnar fovea. Repairs performed with this technique are simple and the results achieved seem to be similar to those obtained with conventional open or arthroscopic techniques, although further investigation with an increased number of patients and follow-up are required. It however became our first choice of treatment in European Wrist Arthroscopy Society (EWAS) 2 lesions of the TFCC. ConclusionsInitial experiences with this technique show that the TES procedure is safe and reliable, requires no specific instruments, and is highly reproducible. There is no need for an expensive anti-adhesion mesh or fixation device, making it cost-effective.

AbstractAortobronchial fistula (ABF) is a rare and devastating complication, if left untreated. Its main clinical manifestation is hemoptysis, and there are no defined guidelines for its treatment yet. Herein, we present the case of a 74-year-old male who complained of back pain and hemoptysis. The patient was diagnosed with pseudo-aneurysm and ABF, and he underwent hybrid thoracic endovascular aortic repair. However, hemoptysis recurred. With patch aortoplasty and anatomical lung resection, successful management of ABF was achieved, with no relapse for 5  years.

Abstract BACKGROUND: The application of telehealth for surgical follow-up has gained recent exposure in orthopedic care. Although the results following joint arthroplasty are encouraging, the role of telemedicine for postoperative care following arthroscopic rotator cuff repair still needs to be defined. The goal of this study was to evaluate the safety, efficacy, and socioeconomic benefits of telehealth as a platform for postoperative follow-up. METHODS: This was a prospective, randomized controlled trial that enrolled 66 patients who underwent follow-up in the office vs. via telemedicine for postoperative v...
