The Preventive Ablation of Ventricular Tachycardia in Patients with Myocardial Infarction (BERLIN VT) trial investigators (Willems et  al; Circulation 2020;Epub ahead of print, PMID 32000514) sought to assess the effect of timing of ablation for ventricular tachycardia (VT) in patients with previous myocardial infarction (MI). In this randomized trial conducted at 26 European centers, patients with history of previous MI and left ventricular ejection fraction 30%–50% who presented with life-threatening ventricular arrhythmias necessitating implantable cardioverter–defibrillator (ICD) implantation were randomized to undergo VT ablation before ICD implantation (preventive group) or after 3 ICD shocks (deferred group).

There are limited data on arrhythmias in acute myocardial infarction with cardiogenic shock (AMI-CS). Using a 17-year AMI-CS population from the National Inpatient Sample, we identified common arrhythmias – atrial fibrillation (AF), atrial flutter, supraventricular tachycardia, ventricular tachycardia (VT), ventricular fibrillation (VF), and atrioventricular blocks (AVB). Admissions with concomitant cardiac surgery were excluded. Outcomes of interest included temporal trends, predictors, in-hospita l mortality, and resource utilization in cohorts with and without arrhythmias.

ConclusionDex conferred anti-arrhythmic effects in the context of ICM via upregulation of Cx43 and suppression of inflammation and fibrosis. The anti-arrhythmic and anti-inflammatory properties of Dex may be mediated by phosphorylation of AMPK and subsequent suppression of NF-κB activation.
