Tracheo-innominate artery fistula with continuous bleeding successfully treated through the suprasternal approach: a case report

Tracheo-innominate artery fistula (TIF) is a rare but fatal complication occurring after tracheotomy. Brachiocephalic trunk transection, one of the surgical treatments for TIF, is mostly associated with a full...

Abstract Anaplastic thyroid cancer (ATC) is an orphan disease with extremely poor prognosis. In particular, unresectable stage IVC ATC is extremely difficult to treat and is associated with a survival of only a few months, even when treated with irradiation and/or chemotherapy. In 2015, lenvatinib was approved for the treatment of ATC in Japan. The aim of the present study was to evaluate the efficacy of lenvatinib for stage IVC ATC. A total of 32 patients with pathologically confirmed stage IVC ATC who were treated at the Kanagawa Cancer Center between 2011 and 2018 were included in the present study, of whom 16 ...

We report our 10-year experience of performing presternotomy ECC for cardiac reoperation and the clinical results. Fifty-seven consecutive cardiac reoperations involving resternotomy were performed between January 2006 and December 2015. ECC was established prior to median sternotomy in all patients. Two patients sustained injury to the right ventricle during sternotomy. Eleven patients sustained injury to the mediastinal structures during dissection (right atrium in 3; superior vena cava in 2; inferior vena cava in 3; left internal thoracic artery in 1; and saphenous vein graft in 2 patients). Longer ECC time and greater ...

AbstractFailure to manage bleeding in the airway is an important cause of airway-related death. The purpose of this narrative review is to identify techniques and strategies that can be employed when severe bleeding in the upper airway may render traditional airway management (e.g., facemask ventilation, intubation via direct/video laryngoscopy, flexible  bronchoscopy) impossible because of impeded vision. An extensive literature search was conducted of bibliographic databases, guidelines, and textbooks using search terms related to airway management and bleeding. We identified techniques that establish a definitive a...

Conclusions Overall, a majority of patients recovered well postoperatively with minimal complications and low rate of reoperation. Our research provides a foundation to develop a risk-stratified approach to determine the need for an ICU admission or early transfer to floor care.

This study assessed outcomes of open and endovascular repair of this disorder.MethodsBetween 1997 and 2016, 152 adult patients presented with KD at Cleveland Clinic (Cleveland, OH), 87 had no intervention, and 65 underwent open repair (n = 55; 19 elephant trunk [ET] procedures [9 with aortic dissection], including 7 frozen ET, and completions with 5 thoracic endovascular aortic repairs, 2 open, and 5 lost to follow-up) or endovascular procedures (n = 10). Non-ET open KD repairs consisted of resection (n = 15), interposition graft (n = 16), patch (n = 4), or aortopexy (n = 1). Maximum KD diamet...

Purpose: Tracheostomy (TT) is considered as beneficial in long-term mechanical ventilation. Little is known on the rate of complication of TT on compared to after venovenous ECMO.Methods: We analyzed 616 patients with acute respiratory failure on ECMO from 2008-2017. TT was performed on or after ECMO. Cumulative complications were analyzed for two consecutive days after TT. Major/minor bleeding was defined as drop of hemoglobin of ≥/

Purpose: Patients on venovenous ECMO are sometimes in need of tracheostomy (TT). Little is known on the rate of complications of a percutaneous (pTT) or a surgical (sTT) approach. Methods: We analyzed 619 patients on ECMO from 2008-2017. TT was performed pTT or sTT. Cumulative complications were analyzed for two consecutive days after TT. Major/minor bleeding was defined as drop of hemoglobin of ≥/
