Scurvy: Remember to obtain a dietary history in inflammatory “rheumatic” conditions

In this study from Seattle Children's Hospital, Perkins et al reported on 10 patients with scurvy who presented to the rheumatology service with symptoms and signs mimicking various rheumatic conditions, such as juvenile idiopathic arthritis, vasculitis, and chronic non-bacterial osteomyelitis. All patients had increased inflammatory markers and all tested patients had MRI findings of bone marrow edema and periostitis, resembling osteomyelitis. This resulted in a 3-month median delay of the correct diagnosis.
