Case Report: An Adolescent Girl with Isolated Neuropsychiatric Features and Apparent Post-Malaria Neurological Syndrome. Am J Trop Med Hyg. 2020 Feb 17;: Authors: Panda PK, Sharawat IK, Panda PK Abstract The post-malaria neurological syndrome (PMNS) is an unusual and relatively underreported complication of malaria, which usually occurs after the resolution of acute febrile illness and the patient is free from parasitemia. The clinical spectrum of the PMNS varies from acute-onset cerebellar ataxia to significant encephalopathy with focal deficits resembling acute disseminated encephalomyelitis. Uncommon presentations of PMNS include Guillain-Barre syndrome, postural tremor, or even isolated neuropsychiatric features. Although in a significant proportion of PMNS cases clinical resolution occurs with conservative treatment only, corticosteroids have been used in an attempt to hasten recoveries. Here, we present a case of a 12-year-old girl with acute onset, isolated neuropsychiatric features, following Plasmodium falciparum malaria. Neuroimaging, clinical examination, and cerebrospinal fluid studies were within normal limits. The child recovered completely after treatment with methylprednisolone pulse therapy. This case report illustrates the need for creating awareness about this uncommon complication of malaria. Conclusions. Liposomal delivery of prednisolone improved renal bio-availability, increased perfusion and reduced cellular infiltrate in the allograft, when compared with conventional prednisolone. Clinical studies should reveal if treatment with LP results in improved efficacy and reduced side effects in patients with renal allograft rejection.

Modern regimen using corticosteroids (CS), tacrolimus and MMF reduced acute rejection rates after heart transplantation (HTx), but has not translated into improved long-term outcomes, potentially due to the complications of the drugs themselves. Therefore, many centers have tried to avoid or minimize CS, and in 2010 we adopted a weaning protocol, changing from a high dose CS regimen (1 mg/kg/day declining in the first month), to a low dose approach (prednisone 20 mg daily on day 3 with weaning within 9 months).
A large number of heart transplant patients are weaned off corticosteroids within the first year after heart transplantation. These patients are selected as they do not have rejection or graft dysfunction. In addition, these patients have not developed donor-specific antibodies (DSAs) and therefore are viewed as being immune-privileged. However, it has not been well established whether these patients have enhanced 5-year outcome compared to patients who remain on prednisone.
The development of donor specific antibody (DSA) after heart transplantation has been associated with increased development of cardiac allograft vasculopathy (CAV). Steroid weaning has been popular among heart transplant programs, however, it is not known whether steroid free regimens present an increased risk of the development of DSA. Orthotopic left lung transplantation (LTx) in mice is an accepted technique to model human post-LTx complications. However, in depth characterization and proper tools to assess chronic rejection (CR) in this model are lacking. Therefore, we developed a multimodal approach to monitor and characterize CR in murine LTx using a major genetic mismatch of donor and recipient under daily immunosuppression (Cyclosporine A (CsA) + Methylprednisolone (MP)).
