Diaphragm dysfunction due to phrenic nerve injury following cardiac surgery has been attributed to surgical trauma or topical hypothermia. The close proximity of the left phrenic nerve to the adjacent left ventricle makes it particularly vulnerable to insult following left ventricular assist device (LVAD) explantation during heart transplantation (HT). ConclusionsIn this study, the authors have successfully established the detailed procedure for GC –MS/MS analysis of midazolam and propofol in human plasma. The sensitivity and specificity of the present method for the drugs were almost comparable to those by the standard liquid chromatography–tandem mass spectrometry. To their knowledge, this is the first report for analysis of midazolam/pr opofol by GC–MS/MS.

ConclusionsOn rare occasions, LHIS can arise from outside the interatrial septum. An LHIS can be differentiated from a lipoma by the presence of entrapped cardiac myocytes in LHIS, making it a pathological, rather than an anatomic, diagnosis.
