However, patients with advanced neuroblastoma, metastatic Ewing's sarcoma family of tumor (ESFT), and metastatic osteosarcoma continue to have an extremely poor prognosis. Therefore novel therapeutic strategies are urgently needed to improve their survival. Apoptotic cell death is a key mechanism for normal cellular homeostasis. Intact apoptotic mechanisms are pivotal for embryonic development, tissue remodeling, immune regulation, and tumor regression. Genetic aberrations disrupting programmed cell death often underpin tumorigenesis and drug resistance. Moreover, it has been suggested that apoptosis or cell differentiation proceeds to spontaneous regression in early stage neuroblastoma. Conclusion The patient-based sensitivity, specificity and accuracy of WB MRI including DWI were comparable with those of PET/MRI. On the other hand, the lesion-based sensitivities of WB MRI were relatively low, mainly because of the low detectability of small lymph node metastasis. The combination of T2WI and DWI showed acceptable detectability; however, the T1WI sequence showed no additional value to detect malignant lesions on both lesion- and patient-based sensitivities.
ConclusionThere is significant diversity of practice in pediatric palliative RT. Combined with ongoing research characterizing treatment response and toxicity, these data will inform the design of forthcoming clinical trials to establish effective regimens and minimize treatment toxicity for this patient population.
