Impact of a formal advance care planning program on end-of-life care for patients with heart failure: Results from a randomized controlled trial
Despite advent of new therapies, heart failure (HF) remains a leading cause of death worldwide.1 Quality end of life (EOL) care for HF patients involves providing care consistent with their preferences. Advance care planning (ACP) discussions with HF patients can facilitate informed decision making, reduce decisional uncertainty and enable care consistent with preferences. Despite the potential, evidence on its effectiveness is limited to oncology patients2 and those at risk of imminent mortality such as elderly hospitalized or in nursing homes.
