Palliative Care in Rural Settings: Closing the Gap (TH312)

Although palliative care programs are present in many large hospitals, access and dissemination of the advances taking place in the field of palliative care to patients living in rural communities have been limited. Patients may receive care from palliative care specialists and return home to communities with vastly different resources. Challenges for rural clinicians include collaborating with colleagues in primary palliative care, developing sustainable business and clinical models to support delivery of complex care, and ensuring effective transitions of care across all settings.
