Utilizing Shared Medical Appointments to Increase Advance Directives Discussion and Documentation (QI607)
Advanced care planning and ADs are part of routine patient care. SMAs are clinical visits in which a group of patients, sharing a common illness or demographic, meet with providers in a unified session, with the goal of discussing relevant issues in a safe environment. SMAs can also be implemented for discussions regarding goals of care, such as advanced care planning, which is often a difficult conversation to broach.
