Implementation of a Modified Memorial Symptom Assessment Tool in the Pediatric Palliative Care Clinic (QI631)

A major goal of pediatric palliative care is to improve patients' quality of life by decreasing their symptom burden. There is limited data on the prevalence of various symptoms in the general population of pediatric palliative care patients followed in the clinic setting. In addition, there is no single tool that is being used consistently across multiple institutions. Without the use of a standardized approach, patients and parents often underreport symptoms and symptoms go untreated.
