Applying the Developmental-Contextual Model to Understand Shared Decision Making During Advance Care Planning: A Qualitative Dyadic Analysis (S760)

Home health patients have increased rates of hospitalizations and emergency department visits, yet have lower rates of advanced care planning (ACP) compared to other patient populations receiving long-term assistance with care. As patients decline, they increasingly rely on proxy decision makers to communicate end of life treatment preferences. Shared decision making among patients and caregivers may reduce conflict when disagreements about goals for care arise.
