Decisions about research priorities are made based on what are thought to be the most pressing scientific questions, in the context of current knowledge and perceived advances. However, research moves slowly while real-world experience accumulates and new approaches are introduced. Adoption of new practices before they have been adequately compared to current treatments are threats to successful conduct of the studies and may expose patients to what is ultimately learned to be inferior treatment.
