Ankylosing spondylitis (AS) is a seronegative inflammatory disease that progressively affects the spine and sacroiliac joints and is more common in men with the B 27 human leucocyte antigen. This leads to inflammation, synovitis and, eventually, secondary arthritis in the hips of those affected. Hips of those affected are often completely ankylosed at time of presentation to the orthopedic surgeon. Total hip arthroplasty offers a stable mobile hip for patients who are functionally disabled due to their debilitating disease. As disease progresses, the native hip anatomy, which is familiar to orthopedic surgeon in routine to...

Publication date: April 2020Source: Journal of Functional Foods, Volume 67Author(s): Nurhan UnusanAbstractGrape seeds are rich sources of proanthocyanidins, which comprise polyhydroxyflavan oligomers or polymers. The beneficial health properties of grape seed proanthocyanidins are attributed to their conjugated and colonic metabolites. There is potential for a two-way relationship between the gut microbiota and grape seed proanthocyanidin. In particular, numerous in vitro and in vivo studies have demonstrated that grape seed proanthocyanidins appear to exert pharmacological effects. These include anti-oxidant, anti-microbi...

This study aimed to elucidate any connection between central sensitization and ongoing pain after knee replacement surgery and other associated factors. Methods: The population was composed of 182 patients who had undergone TKR, and in follow-up visits from 1 mo to 2 yr after the operation, they were evaluated using the Pressure Pain Threshold (PPTs), the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), the Central Sensitization Inventory (CSI), the Leeds Assessment of Neuropathic Symptoms and Signs (LANSS), the Beck Depression Inventory (BDI), and the painDETECT test. Results: Th...

Background: Total hip arthroplasty (THA) is an effective treatment for symptomatic osteoarthritis after failed nonsurgical therapies. Minimizing perioperative complications is critical for faster recovery and shorter hospital stay. The primary aim of this study was to identify predictors of perioperative major complications (MC) and mortality after THA, and to use these predictors to develop a risk scoring system. Methods: The 2006-2016 National Surgical Quality Improvement Program (NSQIP) database was queried for adult patients who underwent primary elective THA performed under general or neuraxial anesthesia. Univar...

Editor's note: From its first issue in 1900 through to the present day, AJN has unparalleled archives detailing nurses' work and lives over more than a century. These articles not only chronicle nursing's growth as a profession within the context of the events of the day, but they also reveal prevailing societal attitudes about women, health care, and human rights. Today's nursing school curricula rarely include nursing's history, but it's a history worth knowing. To this end, From the AJN Archives highlights articles selected to fit today's topics and times. In this month's article, from the May 1925 issue, nurse Ellen G...

Authors: Ingawale DK, Mandlik SK Abstract Inflammation is a physiological intrinsic host response to injury meant for removal of noxious stimuli and maintenance of homeostasis. It is a defensive body mechanism that involves immune cells, blood vessels and molecular mediators of inflammation. Glucocorticoids (GCs) are steroidal hormones responsible for regulation of homeostatic and metabolic functions of body. Synthetic GCs are the most useful anti-inflammatory drugs used for the treatment of chronic inflammatory diseases such as asthma, chronic obstructive pulmonary disease (COPD), allergies, multiple sclerosis, te...

ConclusionsWhile higher income protects Whites with knee OA against overweight/obesity, this effect is absent for Blacks with knee OA. Clinicians should not assume that the needs of high-income Whites and Blacks with knee OA are similar, as high-income Blacks may have greater unmet needs than high-income Whites. Racially tailored programs may help reduce the health disparities between Whites and Blacks with knee OA. The results are important given that elimination of racial disparities in obesity is a step toward eliminating racial gap in the burden of knee OA. This is particularly important given that overweight/obesity i...

AbstractMultiple head-to-head trials have demonstrated that topical nonsteroidal anti-inflammatory drugs (NSAIDs), including topical diclofenac, provide at least equivalent analgesia, improvement in physical function, and reduction of stiffness compared with oral NSAIDs in osteoarthritis and have fewer systemic adverse events. While efficacy of topical diclofenac in osteoarthritis is well established, understanding of the time to onset of action, duration of effect, and the minimum effective concentration is limited. Factors likely to influence these parameters include drug penetration and localization. Diclofenac concentr...

AbstractGlycosaminoglycans (GAGs) are major components of cartilage extracellular matrix (ECM), which play an important role in tissue homeostasis not only by providing mechanical load resistance, but also as signaling mediators of key cellular processes such as adhesion, migration, proliferation and differentiation. Specific GAG types as well as their disaccharide sulfation patterns can be predictive of the tissue maturation level but also of disease states such as osteoarthritis. In this work, we used a highly sensitive liquid chromatography-tandem mass spectrometry (LC-MS/MS) method to perform a comparative study in ter...
