Insertion of an implantable cardioverter-defibrillator (ICD) typically requires both local and systemic anesthesia. In patients who are implanted with a subcutaneous ICD (S-ICD), anesthesia requirements may be greater because of the size and depth of the generator incision and the need for an addition sternal incision. Peri- and postprocedure anesthesia with narcotics can be associated with side effects and dependency. Scaglione et  al (DOI: https://doi.org/10.1016/j.hrcr.2019.12.008) shared a case of a 44-year-old man with Brugada syndrome with a spontaneous type 1 ECG and syncope.
