Gastrointestinal (GI) bleeding remains a major cause of readmission after left ventricular assist device (LVAD) implantation, occurring in 17% to 40% of patients.1 The recurrent and prolonged hospitalizations associated with GI bleeding, including repeat endoscopies and blood transfusions, adversely affect quality of life. Gastrointestinal angiodysplasia (GIAD) remains the main cause of bleeding in LVAD patients, accounting for at least one third of cases. Vascular endothelial growth factor (VEGF) has been shown to be highly expressed in intestinal angiodysplasias and to be directly involved in the pathogenesis of GIAD2.

Rescuing heterotopic heart transplantation (HHT) patients remains a challenge in the field. When a 41-year-old patient was transferred to our service, we chose a novel approach that is reported herein. The HHT had been performed over 20 years ago, and in 2016, because of complications, the patient was listed for transplant with a 1A status. This study shows that CA are released from periventricular and subpial regions to the cerebrospinal fluid and are present in the cervical lymph nodes, into which cerebrospinal fluid drains through the meningeal lymphatic system. CONCLUSION: The Korean urgency classification for LTx is determined by using very limited parameters and may not be a true reflection of urgency. Status 0 patients seem to have poor outcomes compared to the other urgency status patients, despite having the highest priority for donor lungs. Further multi-center and nationwide studies are needed to revise the lung allocation system to reflect true urgency and provide the best benefit of lung transplantation. Lung transplantation surgery often relies on the use of intraoperative extracorporeal membrane oxygenation (ECMO) and necessitates the need for high dose anticoagulation. Heparin induced thrombocytopenia complicates intraoperative anticoagulation management during lung transplant surgery requiring ECMO. Though other anticoagulants such as argatroban and bivalrudin are utilized for the treatment of Heparin Induced Thrombocytopenia (HIT), the lack of reversal agents makes it difficult to use these agents intraoperatively in cases with high bleeding risk.
