Does the nostril shape change after Le Fort I advancement in patients with unilateral complete cleft lip?

Patients with unilateral cleft lip and palate (UCLP) may require Le Fort I advancement to correct maxillary hypoplasia after reaching skeletal maturity. Underlying cleft anatomy, previous operations, and scarring may affect nostil changes following maxillary advancement. The purpose of this study was to determine whether Le Fort I advancement affects nostril configuration (width, axis, and shape) in patients with UCLP. The specific aims were to (1) compare cleft and non-cleft nostrils in patients with UCLP after maxillary advancement and (2) compare changes in nostril configuration in patients with UCLP to those in non-cleft controls after Le Fort advancement.

ConclusionsUnsurprisingly, both the inpatient and outpatient burdens of care are influenced by the complexity of the cleft and increase with worsening severity. Our results will help inform parents of the likely trajectory their child will face when diagnosed with each subtype of cleft. ConclusionA sustained and consistent visiting team approach providing repair of cleft lip and palate defects has resulted in considerable economic gain for Timor Leste over an 18-year period. The training of a local surgeon and multidisciplinary team with ongoing support to the in-country cleft service is expected to reduce the cost per DALY averted once the surgeon and team are able to manage clefts independently.
